  3. Ensure the teacher completes the form and sends it back to our clinic at least 48 hours prior to your child's evaluation appointment.

The Vanderbilt Parent Rating Scale allows you, as the parent or guardian, to provide insights into your child's behavior, attention capabilities, and overall functioning at home and in social situations.

  2. Carefully read the instructions and complete the form based on your observations of your child.

  3. Bring the completed form to your child's ADHD evaluation appointment or mail it to our clinic at least 48 hours prior to the appointment.
